Reply to a provisional admission offer

If you have applied to the UCSF School of Pharmacy and have received an offer of provisional admission, use the links on this page to reply.

Please review all of the information on this page and follow the instructions carefully to ensure that your space in the incoming class is reserved.

Provisional admission vs. final admission

All admission offers are initially “provisional.” An offer becomes “final” in early summer after our office:

  1. Verifies that you have successfully completed all pharmacy prerequisites.
  2. Receives official copies of all transcripts for all college work you have completed. (This includes official copies of transcripts you may have earlier submitted toPharmCAS.)
  3. Receives notice from the UCSF Office of the Registrar that you have submitted the paperwork required to register. (Details are mailed to you after you submit your acceptance below.)
  4. Successfully clears a background check administered by Certiphi Screening, Inc. (UCSF reserves the right to rescind an offer of admission to any individual whose background check reveals unprofessional conduct inconsistent with the high standards of the pharmacy profession or issues that may preclude an individual from participating in Introductory Pharmacy Practice Experiences (IPPEs) or Advanced Pharmacy Practice Experiences (APPEs). Under these circumstances, the Admissions Committee will review the application and provide a recommendation to the Vice Dean for Education. The Vice Dean for Education may revoke an offer of admission based upon this review. The decision of the Vice Dean is final and is not subject to appeal.)

Reply deadline: outlined on our offer letter

You must reply no later than the date indicated on your offer letter or the offer will be withdrawn, you will no longer be eligible for admission, and your application will be canceled.

Offer limited to this cycle

This offer is for this cycle only and may not be deferred. Should you decline this offer and, at a later date, decide that you still wish to attend the UCSF School of Pharmacy, you will have to submit a new application in a future open admissions cycle. Furthermore, this offer is no guarantee that you would be offered admission in a future cycle.
