Decline a Provisional Admission Offer

Use this form to decline when you reply to a provisional admission offer.

STOP! You are about to authorize a change to your admission status that cannot be reversed. Before completing this process, please take a moment to read the following:

We understand that there are many reasons why you might decline an offer of admission, and we have no wish to pressure you into selecting UCSF over your preferred institution. Often, however, some decline this offer because of recent developments or circumstances in their personal lives, and they believe these circumstances will make it impossible to attend UCSF. If such circumstances are the reason you are planning to decline the offer, please consider contacting one of the advisors in our office. We are often able to find solutions to these problems that will enable you to accept the offer and join us here at UCSF in July.

Please consider your decision carefully before submitting your reply. Once you submit your reply, your application will be canceled and cannot be reinstated for this year’s admission. If at a later date you would like to be considered for admission, you would have to reapply in a future cycle. Please note that your provisional admission offer in this cycle is no guarantee that you would receive the same offer in a future cycle.
